___________________________ herbe certified _____  Squire COMBS, late a Private of Captain JAMESON's Company ______ Regiment, Infanty, Kentucky, Volunteers, in the War of 1861, ___ was enrolled into the Service of the United States, at Boonville, in the State of Kentucky, on the 24th day of September A.D. 1861, ___ the period of three years, and was discharged from said Service __ about the 3th day of May 1864, by reason of Guns Shot ___  That during the service of said Squire COMBS he became ___ from performing the duties of a Soldier, from on or about the 19th day of September A.D. 1863, in consequence of a Gunshot wound in the head, received from the Rebels in the service aforesaid and whilst in the line of his duty, and in the manner and at time and place, as follows:  That on the 19th day of September 1863 while engaging the enemy at Chicamanga, in Georgia, He received __ Gun shot wound, which entered the right temple and passed __ at the left temple, and which wound has caused him to be totally Blind, that said Soldier was in good health at the time he entered The service, and continued to until he received said wound.

On this 3th day of September in the year 1864, before ____ a justice of the Peace within and for the County and State aforesaid personally came and appeared Joseph McGUIRE, __ being duly sworn and his oath , declares that the facts _____________ by him made and signed, are just and true so help him God.  That he is entirely disinterested in his statements and ----tify that I have no interest in this matter.
